Scholarship Reimbursement

If you were awarded an MVCTC scholarship, you can now request reimbursement by submitting the required form. This applies to recipients of the following scholarships:

• National Technical Honor Society (NTHS) Scholarship

• MVCTC Foundation Department Scholarships

• Employees’ Association Scholarship

• Treva & Robert Sidle Scholarship

• MVCTC Superintendent Scholarship

To claim your scholarship:

1. Download the PDF form linked below.

2. Complete all required fields on the form.

3. Mail the form to:
   MVCTC Treasurer
   6800 Hoke Road
   Englewood, OH 45315

Once the completed form is received, the scholarship funds will be sent directly to the college or university listed on your application. Please double-check all information to ensure timely processing.
