MVCTC School Health Services

MVCTC is committed to supporting student wellness through dedicated School Health Services. Our goal is to protect and promote student health so all learners can thrive academically, physically, and emotionally.

The MVCTC Clinic is located in Room E103 and is staffed by our full-time school nurse, Mrs. Lanna Anspach.

Clinic Guidelines

  • Clinic Hours: Open during regular school hours.

  • Visiting the Clinic: Students must receive a pass from their instructor before reporting to the clinic.

  • Feeling Unwell? Students should visit the clinic before contacting a parent or guardian for dismissal.

  • If a student is too ill to remain at school, the school nurse will contact the parent/guardian to arrange pick-up or confirm permission to drive home if applicable.

Required Annual Forms

Each school year, all students are required to have an updated Emergency Medical Authorization and Contact Verification form on file. This is vital for ensuring timely communication in the event of a health or safety issue. MVCTC shares these records with the student’s partner school.

Failure to complete the forms by the designated deadline may result in disciplinary action and limited participation in certain lab or school activities.

To complete the required forms:

  1. Visit: www.mvctc.com/emf

  2. Enter the student’s last name and MVCTC six-digit student ID number.

  3. Create a secure password.

  4. Complete Part I: Emergency Medical Authorization

  5. Complete Part II: DASL Contact Verification

    • Include accurate student and parent/guardian contact details.

    • Use a personal student email (not a school email).

    • Ensure that emergency contact names and phone numbers are up to date.

    • Contact MVCTC Student Services at 937-854-6257 for any address updates.
